In order to minimize contact and expedite the check-in process for new patients, we thank you for doing your best to fill these forms out in advance. They may be emailed to frontdesk@westedgedental.com (preferred) or delivered at check-in. We are happy to print out forms for you upon arrival if this is not possible.

Financial Information
It is our mission to provide you with the highest quality of care. If you have any questions about financing or payment, please ask us. We will thoroughly explain your choices and work hard to accommodate your needs. And finally, please be assured that we always discuss all costs associated with treatment prior to beginning any procedure, including your very first appointment!
Insurance
If your insurance provides a benefit for treatment, you will receive the benefit of reduced personal costs. You can utilize your benefits at our office regardless of in or out of network status, and as a courtesy to our patients, we will file all claims on your behalf. Prior to beginning any treatment, you will receive a good faith estimate of how much your insurance will contribute to your care.
Membership Club
At West Edge Dental, we designed our own Subscription Membership Club to be more cost effective and valuable than dental insurance. As opposed to insurance company incentives (profit) our club truly has our patients’ best interests in mind. Membership features zero out of pocket costs for all preventative care and limitless savings with no restrictions. See below for quick fact information and a breakdown of plan options. We understand that choosing between an insurance plan or membership club can be stressful and confusing. Our team members are available and willing to help guide anyone interested through the decision-making process.
